MDS Rating Scales Permission Request Form
The International Parkinson and Movement Disorder Society (MDS) is committed to providing valid and reliable Rating Scales to the scientific and medical community. The Rating Scales listed below are copyrighted materials owned or managed by MDS. All individuals and organizations looking to use these materials must agree to the terms of use, and obtain express written permission from MDS prior to using the Rating Scales.
Permissions to use an MDS Rating Scale are granted on a single use, per language basis, and on a per study, per protocol, per language basis if being used for research purposes.
Submitting a permission request form with your intended use statement is the first step to acquiring the proper permissions to use an MDS-owned Rating Scale. Please select your scale(s) of interest along with the appropriate category of use. A study protocol number or other study-specific identifier such as an IRB, ethics committee number, or a trial registry number is required for requests to use the scale within research. Depending on the type of use, a licensing agreement may be required and licensing fees may apply. MDS' standard license covers permissions to use rating scales in original paper format only. Rating scales may not be modified in any way, including adapted for use on any e-platform, without the express written permission of MDS. Electronic development proposals must be reviewed and approved by MDS before development can begin. Please reach out to the MDS Secretariat for additional information on use of MDS-owned rating scales in an electronic or modified format.
This form will provide an estimated total licensing fee per scale, per use. Licensing fees may be subject to change following MDS' review. Kindly note licensing fees are applied on a per scale, per use, per language basis. Refer to the MDS Rating Scales Pricing Chart for additional pricing details.
