MDS-UPDRS: Request Invoice for Clinical Trial
The MDS-UPDRS: MDS-Unified Parkinson's Disease Rating Scale Certificate Program is offered to industry-sponsored clinical trials with prior written permission from MDS only.
Clinical trials must obtain a license from MDS to administer the MDS-UPDRS. Licensing fees will apply on a single-use, per-study, per-protocol, per-language basis. The cost of training registration is $1,000 USD for MDS members and $1,500 USD for non-members. Training registration fees are separate from licensing fees. If a license has not yet been obtained, we ask that a study representative submits a Permission Request Form to initiate the licensing process.
Please note that a rater training contract with the sponsor or CRO is required in most cases. Rater training requests should be submitted by a central project management team. Individual raters should contact their site supervisor.
Project managers should fill out the form completely and click "Submit" at the bottom to request training on behalf of an industry-sponsored clinical trial. Requests will be processed within 5 business days.
