MDS-UPDRS: Request Invoice for Personal/Educational Use

The MDS-UPDRS: MDS-Unified Parkinson's Disease Rating Scale Certificate Program is offered to clinicians and health professionals that are not taking the training as part of any research study or clinical trial. To participate in the training program, please fill out the form completely and click "Submit" at the bottom when you are finished. Requests will be processed within 5 business days and the invoice will be sent to the e-mail address provided.

Please note that full payment must be received by MDS prior to accessing the training. Once payment is received you will receive an e-mail confirmation with instructions for logging in to the MDS-UPDRS: MDS-Unified Parkinson's Disease Rating Scale Certificate Program.
